Waiver
2025 CarmelFest Volunteer Waiver and Release of Liability
Please read carefully before signing.
In consideration of the opportunity to volunteer at 2025 CarmelFest, I voluntarily agree to assume all risks related to my participation. These risks may include, but are not limited to, property damage, personal injury, or death.
On behalf of myself and my heirs, executors, administrators, and assigns, I hereby fully release and hold harmless the following Released Parties:
Carmel Rotary Club, the CarmelFest Executive Committee, all affiliated organizations, event sponsors, vendors, fellow volunteers, employees, officers, directors, agents, and representatives—from any and all claims, liabilities, damages, or expenses arising from or connected to my participation in CarmelFest activities, regardless of cause, including any resulting from negligence.
I also agree to indemnify and defend the Released Parties from any claims or lawsuits brought against them due to my actions while volunteering.
I understand and confirm the following:
1. I am in good physical health and capable of participating in volunteer activities.
2. I authorize medical treatment for myself at my own expense if needed.
3. I grant permission for Carmel Rotary, sponsors, media, and marketing partners to use my name, image, voice, and likeness in any form of promotional or event-related media, without compensation or restriction.
Communications Consent:
By submitting this form, you agree that CarmelFest, the Rotary Club of Carmel, and their affiliates may contact you via email, text, or phone regarding event updates, volunteer opportunities, community initiatives, fundraising efforts, promotions, news, and any other communications related to our organization. You may opt out at any time by following the instructions in any message.
For Volunteers Under 18:
A parent or legal guardian must also sign this form to provide consent for minor participation and to accept the above terms on the minor’s behalf.
This waiver is intended to be as broad and inclusive as permitted by law. If any part of it is held invalid, the remaining sections shall continue in full force and effect.
